The exam tests candidates’ pre-admission knowledge of areas in their programs. How to Register for the HESI A2 Exam

Candidates can take the HESI A2 at designated testing sites and at colleges and universities. Prometric testing centers offer the exam on site or online. Test-takers create an Evolve account on the Elsevier site and register for a distance exam according to their school’s instructions. Students will need to enter a code specific to their school.

Registering for testing at a college or university requires calling the school to schedule or register online and creating an Evolve account. Test fees may vary by institution. Some schools offer online HESI A2 exams. Students take the test on their own computers or in a school’s computer lab and register according to their nursing program’s instructions.

What is the HESI?

What’s the test all about? Well, let’s start with the name. HESI stands for Health Education Systems, Inc., and A2 stands for Admission Assessment. The test is also known by a wide variety of other names, such as the Evolve Reach, the Evolve Reach A2, and the Evolve Reach HESI. However, most people just call it the HESI test. It’s not the name that’s so important about this exam, but what it does.

As part of their application process, many nursing schools across North America use the HESI A2 as an entrance examination. It’s a diagnostic tool, but it’s also a screening tool. Nursing schools have always received far more applications every year than they can possibly approve, and the problem has been getting increasingly worse for the past few decades. Because of the steadily rising population in the United States, the demand for healthcare workers in all fields is growing, including nursing.

The other factor is the HESI exam. A set of great HESI scores, in combination with an excellent academic record in high school, gives a person an excellent shot at getting into nursing school . In fact, excellent scores can help make up for a less than stellar high school GPA. That’s because the test is specifically designed to predict which test takers have what it takes to succeed in a nursing education program.

So when you sit down and finally take the HESI, you want to make sure you’re absolutely prepared to do well, because your results truly can make or break your chances of being accepted.

HESI Exam Outline

The HESI is different from many standardized admissions exams in its format. With most college admission exams, such as the SAT or ACT, every person takes the same basic test. There are different versions, to prevent cheating and for research purposes, but with most standardized exams each test taker will see the same kinds of questions, and will have the same time constraints.

With the HESI, though, colleges and universities have a lot more flexibility. There are eight main components that make up the full exam, and schools can choose which ones they want their applicants to take. Depending on your program, you may only need to take the Mathematics, Reading Comprehension, Vocabulary, and Grammar subtests. Some schools may ask you to take the Biology and Anatomy & Physiology subtests as well, but the Chemistry and Physics subtests are rarely ever required.

The schools can also set their own time limits for each part of the test, although many nursing schools use the time limits recommended by Elsevier, the company that develops the HESI.

These are the eight HESI A2 tests:

• Mathematics
• Reading Comprehension
• Anatomy and Physiology

On each of these sections, there will be five questions that don’t count toward your score. Elsevier uses these five experimental questions for research purposes, with the goal of improving future versions of the HESI. However, you should do your best to answer each question correctly, as you will have no way of knowing which ones are experimental and which ones aren’t.

Many nursing programs require applicants to take all eight HESI sub-tests, but not all do. You’ll want to check with the nursing school you’re applying to in order to find out which tests you’ll need to prepare for. In addition, to the academic subject tests, there are two additional components of the HESI A2 that most applicants will have to take: Learning Style and Personality Profile. These last two aren’t tests of your knowledge; they measure your aptitudes in certain areas deemed important for success in nursing school. (These two are almost always required.)

Finally, although it’s not a part of the HESI A2 itself, there is a companion test that more and more nursing programs are using in addition to the HESI. It’s called the HESI Critical Thinking Exam. You’ll want to check with the schools you’re applying to in order to find out if you’ll need to take this test.

HESI Exam Registration

In most cases, the nursing school or program you’re applying to will handle the actual administration of the HESI exam. It will be up to them to determine which of the various sub-tests you’ll take, what the time limits for the various tests are, and where you’ll take the exam. They’ll also determine the fees you’ll pay to take the HESI A2. In many cases, you’ll take the test right on the college campus.

All of this information should be available on the nursing program’s website. If it isn’t, call or email the nursing department of the school to get the information.

Retaking the HESI Exam

This is another matter that is up to each school to decide for itself. Most schools allow at least one re-take if a person isn’t satisfied with their score. However, there is often a waiting period that must be satisfied before attempting the exam again. Some schools allow more than one retake, but only so many within a calendar period. Ideally, you’ll do your very best on your first attempt and won’t need to worry about retaking the HESI .

HESI Scores

There are no official passing scores or failing scores on the HESI exam. Each nursing school or program sets its own standards for ranking scores on each subtest. Generally speaking, though, any score below 75% is considered to be an indication that the applicant is not academically ready for the challenges of nursing school in that area.

A score in the range of 75% to 79% is considered to be in the bottom range of acceptable, but an applicant whose score falls in this range will probably need quite a bit of help to succeed in nursing school. Scores from 80% to 89% indicate that the applicant should be able to graduate from nursing school with only minimal academic assistance. A score of 90% or higher is a sign that the test taker is very likely to do well in nursing school without any academic assistance.

The Importance of a HESI Practice Test

Making great scores on the HESI A2 can make a huge difference in helping you secure admission into a nursing school program. Conversely, a poor or even so-so showing can ruin your chances of being accepted into a program. While it’s true that in most cases you’ll be able to retake the exam at least once if you do poorly, that doesn’t necessarily mean you’ll do any better the second or third time.

Generally, people who retake the HESI will have an entirely different version of the test on their retakes, and won’t see any questions that they’ve seen before. So trying to go home and review specific questions you’ve failed won’t work. So you’ll need to put in some serious study before taking it again, which can mean missing the start of a semester and being forced to delay the start of your nursing education by several months, or even longer.

The best plan is to review regularly and thoroughly, starting well before your HESI A2 test date, and then, with a few weeks left to go, take a practice test to find out where you stand. This way you’ll be able to discover which areas you need to brush up on while you still have time to make serious improvements. By doing so, you’ll be in excellent shape to make an outstanding score when you take the actual HESI A2.

What is the hesi a2.

The HESI A2 ©  (also known as the Evolve Reach Admissions Assessment) is a common nursing school entrance exam.

What subjects are on the HESI exam?

The HESI A2 ©  consists of 8 separate test areas. Those test areas include  mathematics ,  reading comprehension ,  vocabulary ,  grammar ,  biology ,  chemistry ,  anatomy & physiology , and  physics . The school you are applying to will tell you which of these 8 test areas you will need to take. Almost all programs will require you to take the math, reading, vocabulary, and grammar parts of the test. In addition, some school may require you to take the biology and anatomy sections. It is rare that a school will ask you to take chemistry or physics.

How many questions are on each section?

Here is a breakdown of the number of questions you will face on each section of the HESI exam:

Is it multiple choice?

It is multiple choice except for a couple of math problems which will be fill-in-the-blank.

How much does the HESI exam cost?

The cost to sit for the exam is $40.00.

Where do I take the test?

The program’s admissions counselor will work with you to schedule your exam at a Prometric testing center near you. You will take the exam on a computer and it will be proctored (someone will be there to watch you take it).

What is a passing score?

The program you are applying to has a pre-determined passing score. However, most programs require a 70% – 80% on each test section.

What happens if I don’t pass my exam?

Before taking this test I searched everywhere to try and find out what the critical thinking section would be like. I really couldn't find anything useful in easing my unease about that part of the test, so I thought I might post something to help others who take the HESI with the CT section.

The information I received with my CT scores follows:

This exam is a four-choice, multiple-choice test. All answers are correct. However, each choice reflects a different degree of critical thinking skills.

Scores range from 0-1000. The higher your score is, the better your ability to think critically within the discipline of nursing.

In addition to a total score on this exam you will receive scores on five sub-categories of critical thinking:

Problem Solving: These questions refer to the process of inquiry in which the nurse seeks multiple facts to remove obstacles or resolve patient care problems.

Biases and Ethical Dilemmas: These questions address ethical, legal, and moral issues, as well as biases nurses may have about patients and others.

Argument Analysis: These questions refer to situations in which the nurse seeks to resolve conflicts or disagreements by considering multiple points of view.

Analysis of Data: These questions address the need to interpret patient data correctly, and decide if further nursing action is required.

It says five sub-categories, but it only details four. Prioritization of patient care is not listed, but I think the title is very descriptive. Someone on another post suggested a practical nursing book, but I didn't have a chance to find one and study it. I scored 870 overall on the CT section. Can a nurse accept muffins as a thank you? I know that I myself would want to say thank you with baked goods, but I don't know if that's something that should be done. I also am not certain to what extent an off duty nurse should diagnose a stranger. It's obvious from my scores that I needed more knowledge regarding Analysis of Data and Argument Analysis.

Was that true when you took it?If every choice is correct, do you get partial points for answers that would have been the "next best answer"? no i didn't use any books. the questions they ask you have to basically think if you was in that situation as a nurse and choose the best solution or action to take. it was kind of easy if you know the guidelines and do's and dont's of being a nurse.

Wow, I just took the CT exam, and I'm surprised that the questions (5 years after the original post) seem almost the same. I took the "muffins" (a box of chocolates on my exam) from a patients son, and shared them with the staff because it was a nice gesture, and to refuse seemed offensive. Regarding the question in the pharmacy (while off-duty), I chose to refuse to offer medical advice because I'm not a doctor. I also got the question about the patient defecating on themselves in the bed (chose to clean the patient first, and use towels underneath him until I could find clean sheets).

Honestly, there is no magical way to approach the questions. I just really tried to imagine myself in the situation, and decide what action I would take. For example, if I need to change bed linens for a patient, and the floor was out of linens, I would take them off of an unused bed. If I was in the middle of changing a dressing for a home health patient, and another patient called, I'd silence the call until I was done; however, if I was at dinner with friends, and I was on-call, I'd leave the table to take a call. If a patient just got a cast on their lower leg, and it became very painful, the first thing I'd check is their their toes to make sure there is adequate circulation. Really, all you need to do is think about what you'd do as a professional, and perhaps also think what you'd like a nurse to do for you if you were the patient. However, for the question about the guy who wanted to meet the nurse for coffee to discuss the physician, I chose to "explain professional boundries"--not everything the patient wants is acceptable :-). Overall, I scored 940, not the highest, but good enough. Good luck to all future HESI testers!

How Long Does it Take to Get Results?

You will receive your HESI score shortly after your exam. Your score and a remediation plan are sent to you within two weeks of completing the exam. 

What Happens if You Fail the HESI Exam? 

The HESI exam is not a traditional pass/fail exam. It gives you a numerical score between 100-1000. The higher the number, the better you performed on the exam. 

Your nursing institution may have a score that you must meet for program acceptance. You need to speak with your administrators to see what the minimum accepted score is. 

Most nursing schools require that students take the HESI before graduation to ensure they are ready to take the NCLEX. Many nursing programs require a score of 850 or higher because that indicates you will most likely pass the NCLEX.

If your score is too low, you will need to retake the exam. Before you do, make sure that you study with HESI prep courses that will help you perform better the next time. 

No matter what nursing program you go to, you will need to study to pass your courses and licensure exams. The HESI exams are a great tool to ensure that you have the necessary knowledge to earn licensure and perform as a nurse. My experience with the TEAS/HESI

I took my HESI and my TEAS this month with about a month of studying intensely every day (10-12 hours), and got a 93.6 on my HESI and a 93.3 on my TEAS. It placed me in the 99th percentile on the TEAS for math, science and english, and 95% percentile for reading. I hadn't taken a standardized exam since 2015, and I was so freaked out going into it. This is probably my life accomplishment right here but I just want others to have the same success story so I thought I would make a post about what I did.

Here is my overall break down of each exam:

(Jan 15) HESI: 93.6%

Reading 96%

English Language Use: 94.67%

Anatomy & Physiology: 96%

Critical Thinking: 850 pts

(Jan 20) TEAS: 93.3%

Reading: 91.5%

Math: 96.9%

Science: 91.5%

English/Language: 91.7%

For one, I would take all the practice quizzes and exams you can, get a baseline of what you know and what you don't and go from there. I will post my links throughout the post, but a lot of the websites have HESI and TEAS practice exams, which gives you more questions to practice with versus only taking the practice TEAS questions because you're taking the TEAS. I spent quite a bit of money on study materials, but I also had money from my mom to purchase whatever I thought was necessary to pass on the first try, so I know all of my resources are not feasible for everyone. So this was probably way overkill on what all I bought to review but I just want to list out what I bought and what I thought was useful:

I purchased the Mometrix physical book for the HESI, and then purchased the online course for the TEAS because it came with 9 practice exams for EACH subject. (Please be careful with the science exams they provide as I had a mental breakdown the night before my TEAS because I took practice exam 7 and it had questions about stoichiometry and calculating energy and I had none of that on my exam). If you go to the Mometrix website they have the online courses discounted, and if the code isn't on the website, I would shoot them an email and ask if they have any specials for the online courses. That is what I did and they emailed me a course code to get 25% off. I felt like both of these were very helpful in covering pretty much anything that could be on the exam, along with lots of videos integrated into the material. The online course has them embedded into the page you're reading, and the book provides the website link and code.

ATI Prep: I really considered purchasing their whole prep pack option, but decided I probably wouldn't have enough time to review everything else I had bought or looked up, so I opted to purchase the practice assessment package. It came with assessment B, C, and a practice A&P exam. Take that practice A&P exam every. damn. day. I took it like 7 times (no limit on the times you can retake it), I may or may not have seen some exact questions on my exam. I took the practice assessment B & C a week before my exam and then the day before my exam. They were wonderful in showing exactly how the test would go. Literally the webpage for the practice was exactly like the webpage for the real thing. However you can only take each practice assessment (B & C) twice, so keep that in mind. You can't exit out and finish it later, but it does let you "pause" it for 8 hours. It is also currently 25% off!

Adkins Academy: For this resource, if you can afford it, I 100% recommend it. It is not cheap, I'm not going to try to hide that fact. I'm going to link his youtube and website below for those who are interested. Dr. Adkins was a lifesaver to my confidence going into the exam. However if you can't afford it, he has some wonderful 30 minute youtube videos posted that are little snippits of our online class. He offers a 6 hour long review session twice a week, live. Like you can write in the chat box a question and he answers you by name. He offered great "hacks" on how to quickly answer questions in all topics of the exam, as well as how to identify incorrect answers for questions you are unsure of. He also has a 1.5 hour office hour where you can show up and ask any questions. So overall he offers about 14 hours of live help a week, and you're allowed to be in the program for a year. He has a schedule that repeats every month - Science is broken into 2 classes, Math typically is as well, English and Reading have their own review sessions and then he has a comprehensive review for the HESI/TEAS twice a month. He mails you a question packet of around 60 questions per subject as well as a science review packet where he includes his lecture slides (5 packets total) and you go through the packet together in class. Amazing amazing resource if you can afford it, I will be taking his NCLEX review when I take my NCLEX. Breakdown of the exam:

Science- overall I would say focus on understanding general concepts before trying to memorize specific details. For example - know where the muscles are located on your own body, before trying to memorize what each of them do. Also do NOT sleep on the chem and earth science review for this section. I would have a general understanding of the types of chemical bonds (polar covalent, nonpolar covalent, hydrogen, and ionic bonds) and basic examples of each in the body. Understand general types of heat transfers, chemical reactions, basic basic chemistry stuff. Understand the concept of periodic table trends (and practice identifying them on the table, my chem questions didn't come with a periodic table :/ ). You may think you remember the scientific method for experiments, but I would recommend at least looking it over again as there were quite a few questions about IV, DV, control, correlation, results, ect. However I will say, at least for my test, there was no need to go crazy in depth with the chem. Professor Dave Explains has lots of great videos for free on youtube, and is broken down like a kids show IMO, and makes everything very digestible. However, I took the TEAS 6, and have read on the ATI website and been told by Dr. Adkins that the TEAS 7 (rolls out June 2022) has a lot more chem questions, and less science questions overall, meaning you better get those chem questions right. Crash course has a free mobile app with flash card sets as well. And who doesn't love crash course.

Math- both the HESI and TEAS had an on screen calculator for you to use. The HESI was a 4 function calculator and the TEAS was a lot more complex, it had the square root function and a percent function. Know how to turn decimals into percents and vise versa, know fluid conversions and basic metric conversions (ft to meters, cm to ft, ft to miles) and you'll be okay. Definitely understand proportions and ratios and how to solve them, I actually preferred to use a proportion/ratio set up to solve my conversion problems. Military time, roman numerals and table interpretation might show up so I would have a basic understanding of those. I watched a few youtube videos and took lots of practice exams. I will link what resources I have below. I took Brandon Crafts practice exam and watched his youtube videos that covered what questions I missed, it was super helpful. I would know basic geometry as well, at least triangles, circles, rectangles and squares. Perimeters, areas and volume, at the bare minimum. I highly recommend timing yourself as you take your exams so you can push yourself to be faster without making mistakes because it's about down to a minute a question.

English- I hate english. I don't have any specific suggestions other than take practice exams and quizzes to figure out where you are weak and then move from there. I used a lot of online quizzes from grammarbook. Obviously understand (than / then), (affect / effect), (who / whom), (that / which), and subject verb agreement. I used a quizlet (link below), as well as made my own quizlet (link below) for the vocab words review. Super important to read your questions before answering for the vocab questions. On the practice reviews I would get questions wrong because I would put the definition or synonym without reading what it is asking for. Sometimes it wanted the antonym, or synonym or definition. Know basic spelling of commonly misspelt words, I would recommend you look up the words to accommodate your vocabulary so you can study according to your availability . Sometimes the grammar words were used in a sentence and you were asked the definition/ antonym/ synonym, other times it would give you a word naked and ask. Grammarbook has loads of free exams and quizzes that helped with english rules, because there are a lot we forget about, and just because it "sounds right" doesn't mean it is right.

Reading- obviously read your question before answering it. Sometimes it would ask a question that doesn't require you to even read the whole passage, it could just be about paragraph 3 or whatever. Super important you understand how to identify a topic vs main idea, as well as the styles of writing. Again Dr. Adkins does a really good job explaining this, and has some amazing tricks for this portion if the exam. However, taking practice quizzes, reading the mometrix books and watching youtube videos helped me a ton too.

Critical Thinking (HESI): I did not review for this section as mometrix made it seem like it was going to be about learning styles and if you were an introvert or extrovert. It was not lol. It was very much nursing application questions. I found myself going How am I supposed to know this if I haven't even gone to nursing school yet . Triage questions, ethics questions, decision making questions, assignment questions, it caught me very much off guard. You score on a scale of 1000 points and the closer to 1000 you get, the better "decision maker" you are. I wasn't upset with my 850, but feel like if I had reviewed a little I could have done better.

HESI: did not show me a timer, I don't know if you can wear a watch or not as I didn't, but take some timed tests at home. You are given the calculator for each section. You are allowed to use the time however you want. I finished the A&P section in 8 minutes and took 78 minutes on the math. It gives you your score as you finish each section. I closed my eyes so I wouldn't see it and get disheartened if it was a bad score lol. You are also allowed to pick what sections you want to do in what order, I recommend math first. After you answered a question you could NOT go back and answer it. I took mine at a testing center and I was allowed to get up to use the restroom or access my locker where I brought a snack lol. However the time does keep going . I wouldn't have taken a break at all but my stomach was sounding like Shamu was having a Seaworld presentation. I took it at Prometrics, and we had a cubical so you couldn't see anyone else. I was allowed to bring in ear plugs, but they also provided noise cancelling headphones (I don't see how its very sanitary but if I didn't have my ear plugs I would've used them). I preferred to take it in person so if wifi crashed or something it wasn't my problem.

TEAS: I took it remotely at my house, I thought I had registered to take it in person and realized the day before I was taking it at home. With that being said - do the dry run y'all, I would've been screwed if I had waited until the morning of to make sure all my tech was up to speed. Again if you take the practice exams for ATI it shows you the exact set up of the exam screen. Reading, Math, Science, English, and I was not offered a bathroom break. The exam shows a timer for each section that counts down. Calculator was provided for every section. You do not get to see your score as you finish each section. However you could move freely between questions after answering them, within that section. You can also flag questions to go back and answer, however you MUST un-flag them before submitting or else it won't be graded. You were able to zoom in and out of the screen which was super helpful for me for the math section so everything wasn't so tiny. You also can minimize your camera image that shows you being recorded. I am way to ADD to be seeing myself in the corner of my screen while taking the exam. At the end of my exam there was some technical glitch where I was unable to rip up my scratch paper on the recording, but this did not affect my exam score. I also moved my lips while I talked to myself and would look up to the side to think, and my exam was not flagged, in case anyone is wondering about the proctoring.

I will happily answer any additional questions anyone has, I hope my post helps someone, I know I really tried to search this reddit for posts about the exam and only found a handful (which were super helpful btw). Good luck to anyone taking it, it is definitely possible to pass if you apply yourself and study!
