How do we assess the impact of Health Technology Assessment (HTA)? Whilst high-income countries (HIC) may have led the way, lower-income countries are increasingly beginning to develop HTA processes to assist in their healthcare decision-making. Understanding how we might quantify the costs and benefits of investing in HTA is important to policy makers and donors. Very few studies have, however, estimated the benefits of the process of HTA in terms of its value to the health system. The global expansion of HTA, its variable implementation, the lack of quantified evidence on health outcomes, along with an increasing investment in these processes at the systems level in low- and middle-income countries (LMIC) has generated greater interest from policy makers about the value and return on investment (ROI) of HTA. A lack of longer-term impact assessment (IA) may undermine its importance and value.

To fill this research gap, we have developed a methodological framework to estimate the ROI in HTA using net health benefits (NHB) as our measure of value. This is the difference between QALYs gained by an intervention and QALYs that could have been gained if the money required to deliver it had been spent on other interventions. We use a mixed-methods approach to quantify the value of HTA and to produce explanatory programme theory on the mechanisms by which HTA impact can be optimised. It is also important to consider opportunity costs when establishing HTA processes but which are often overlooked. The aim is to convey the concepts of potential and realised population NHB, and what we can attribute to the HTA process. Central to understanding this is the ‘value of implementation’ (VOImp). Theory-driven approaches will be used to generate and test contextual explanations for gaps between expected and actual gains in population health.

We envisage the use of this research will encourage accountability of spending decisions and help to optimise the impact of HTA in an era of investment and expansion, in particular, for LMICs, through better understanding of HTA’s role in delivering health outcomes and value for money at the system level. This research will offer a forward-looking model that LMICs can point to as a reference for their own implementation.

  • New technologies can be tools of innovative change in healthcare. They can be associated with improved treatment options, quality of care for patients, and cost savings. Distinguishing valuable technologies from those that offer added costs with no improvements in outcomes is the art of technology assessment. A key function of that art involves selecting those patients, conditions, providers and settings in which a technology may offer improvements over current care. With recent scientific discoveries such as the mapping of the human genome, development of genetic marker tests, and the growing interest in stem cell technologies, innovation is far ahead of any type of assessment that is currently used to establish which technologies should be made accessible to patients. Screening technologies are on the forefront of innovation and may have a dramatic impact on the care of patients in terms of identifying disease and appropriate treatment options at an early stage. As a result, screening technologies are of key interest to health technology assessment (HTA) agencies in the United States and abroad. Similarly, because screening technologies are developing quickly and are believed to have the potential to make a significant change in patient care, it is important to develop a robust level of HTA criteria to evaluate these new technologies and determine which technologies should be integrated into the practice of medicine and made accessible to patients. Findings from this study indicate that while technology assessment organizations do have standard sets of criteria to evaluate products that are therapeutic, the assessment and level of evidence used to evaluate screening technologies are less clear. The objective of this research study is to evaluate existing technology assessment standards for screening technologies in order to establish a best practice that may be implemented by US technology assessment organizations to broaden the criteria used in assessments for screening products. The results of this study indicate that the best practices should include criteria to: support screening reliability, sensitivity and specificity; evaluate data to identify appropriate patient populations; reference to the natural course of the disease; consider ethical implications; and the impact of cost.

Title: can gpt-4 do l2 analytic assessment.

Abstract: Automated essay scoring (AES) to evaluate second language (L2) proficiency has been a firmly established technology used in educational contexts for decades. Although holistic scoring has seen advancements in AES that match or even exceed human performance, analytic scoring still encounters issues as it inherits flaws and shortcomings from the human scoring process. The recent introduction of large language models presents new opportunities for automating the evaluation of specific aspects of L2 writing proficiency. In this paper, we perform a series of experiments using GPT-4 in a zero-shot fashion on a publicly available dataset annotated with holistic scores based on the Common European Framework of Reference and aim to extract detailed information about their underlying analytic components. We observe significant correlations between the automatically predicted analytic scores and multiple features associated with the individual proficiency components.
